Self management is the name often given to a set of approaches which aim to enable people living with long term conditions to take control and manage their own health and put them in the "driving seat" of their care.
The Government's Strategy for self management is set out in Gaun Yersel, which was written by those with conditions aided by the Health and Social Care Alliance Scotland (the ALLIANCE), a strategic partner of the Scottish Government in delivering the self management strategy.
The strategy set out the principles of self management, which include making the person 'the leading partner in management of my health' and identified five key stages at which people need support:
- Living for today
- End of life
It also established the Self Management Fund, a £2m per year fund which is managed by the ALLIANCE to provide grants to third sector groups to encourage the sharing of good practice and the development of new approaches to self management.
Information that is up-to-date and personalised - is absolutely essential to proper self management. To address this need the Government developed ALISS (A Local Information System for Scotland), an innovative project which makes information about things which people have found helpful in managing their conditions in their local community easier to find.
GPs and their teams have a central role in enabling people to manage their health and in helping them engage with the supports they need in their communities.
Self management is critical for the Scottish Government's Quality Strategy, which sets out a 2020 Vision for a safe, effective and person-centred health service.
This promotes a shift in the current model away from people being passive, dependent recipients of care to a model that engages, empowers and supports people in a partnership approach with their healthcare professionals, carers and community.
This approach is not a stand alone policy area, and links with person centred planning, primary care, self directed support creating health, as well as the 'clinical priorities' team. It is also closely related to co-production, empowerment, person centredness.
